POSTOPERATIVE CARE

Prophylactic antibiotics are prescribed for up to 2 or 3 weeks. In the first postoperative visit the packing is removed , and this is routinely scheduled on the second or third postoperative day. The patients are instructed to begin instilling Ponaris (Jamol Laboratories, Inc, Emerson, NJ) drops into each nasal cavity three times a day for lubrication. One to two weeks later the middle turbinate gloves or, if still present, the absorbable gelatin packs are removed from the nose, and direct endoscopic visualization and debridement of crust and thickness secretions is instituted on a weekly to biweekly basis until the ethmoid cavities are clean and healed. Daily saline nasal douching utilizing a Water Pik at home is also instituted at this time. Nasal saline pray is also helpful. Occasionally postoperative injections of 40 mg/mL triamcinolone acetonide into persistent maxillary sinus polyps and oral doses of prednisone are necessary to minimize inflammation and speed healing. Triamcinolone accetonide spray may be used to minimize scarring, granulation tissue, and polypoid mucosa is essential for a successful outcome and to prevent the need for subsequent revision surgery.

Care of the external nose is identical to that recommended if rhinoplasty were performed without concomitant sinus surgery. The Aquaplast thermoplastic splint is generally removed about 1 week postoperatively and Steri-Strips are replaced as necessary and left in place for up to 3 weeks in all. Triamcinolone acetonide 10 mg/mL is infiltrated beneath the skin -soft tissue envelope if persistent supratip edema appears likely to predispose to a pollybeak deformity.

I do have insurance, work full time, have for all of my adult life. And, I can’t afford the care of my own freaking teeth!! Whether you have insurance or not, it is a fortune. The health care industry is in crisis, never in older generations did people have to pay so much for health and dental insurance and STILL have to pay a fortune out of pocket and for prescriptions that are expensive. Its what happens when something as important as health care is left to the wolves of capitalism, as is the way of so many of our systems over the last few generations. I would give anything for SOCIALIZED HEALTH AND DENTAL CARE RIGHT NOW. We live in a world where we have to be insured for everything – this is progress?!?!

Virus anywhere in the upper respiratory tract can affect you sense of smell: the olfactory sensory nerves are very sensitive. Saline nasal rinses are unlikely the cause of “spreading virus from your ear to your olfactory area,” but I suppose it’s possible. I have occasionally seen patients (or in my own personal experience) where a sinus infection seems to spread to an ear infection. That MAY have been related to saline rinse moving through an infected area (nasal passages) into the throat (where Eustachian tubes travel to the middle ear), but again: that ‘spread’ of infection may have occurred even if saline rinses were not being done.

In patients where the nasal polyps do not seem to respond to the above protocol (or if patient is refusing surgery), the addition of a very strong drug called Zyflo (zileuton) may be of benefit. This drug can cause liver damage (in <10% of patients) and as such, liver function bloodwork (AST/ALT) is required every month for 3 months and yearly thereafter. As long as the AST/ALT numbers do not exceed 150 (yes, it is higher than what is considered normal), it is generally considered safe to continue with this drug (though would check bloodwork weekly if that happens to ensure stability). Getting an immunologist involved in the care of such patients would also be beneficial, especially for those patients where the nasal polyps are particularly resistant to standard treatment.
